Whipple´s disease with neurological manifestations

The authors present a study case of Whipple´s disease affecting
primarily the CNS in a 69 years old woman, who was admitted
because of movement changes (ataxia and lower limbs paresis)
and cognitive changes (depression, behaviour and personality
changes).
The presence of at least one of the criteria proposed by Louis et
al. in 1996 revealed the presence of Whipple´s disease, which was
the positive result in the cerebrospinal fluid (CSF) by Polymerase
Chain Reaction (PCR).
At present the patient is undergoing treatment with Co-trimoxazole
(800 mg of sulfamethoxazole and 160 mg of trimethoprim), 2
id. The treatment improved considerably the patient´s condition.
Because this disease is potentially treatable, it is extremely important to trace this pathology, especially in patients with atypical
neurological problems or without a correct diagnosis.
