Primary extragonadal germcell tumor with choriocarcinoma? Case report

A germcell tumor usually has a gonadal location, rarely ocurring
isolated in extra-gonadal locations but with a biological behaviour
similar as the former, altough with a worse prognosis.
The diagnosis of a primary-extragonadal germcell tumor with
choriocarcinoma is possible but extremely rare, being only diagnosed after a gonadal germcell tumor is excluded by histological
examination.
The autors describe a case of a 31 year-old man with enlarged
abdominal lymph nodes as well as liver metastatic disease; a
surgical lymph node biopsy was performed revealing a choriocarcinoma TxN3M1bS2. The patient started as soon as possible
chemotherapy with bleomicine, etoposid and cisplatin (BEP) with
clinical improvement.The patient was then submitted to left radical orquidectomy due to alterations revealed by ultrasound; no
neoplasic tissue was found; he’s now in complete remission with follow-up of 24 months after treatment.
