Abstract

The authors describe the case of a 67-year-old man who, after coronary bypass surgery and anti­ coagulation treatment with warfarin, presented with fever, arthralgia, bloody pericardial and bilateral pleural effusions, pneumonitis, anaemia and a prolonged protrombin time. He also showed reduced effort tolerance, distended jugular vein pressure and ankle oedema. All the symptoms disappeared within 3 months. The patient was followed for two years with no subsequent re-appearance of symptoms. The authors conclude that the patient had post­ pericardiotomy syndrome with the serositis being worsened by a haemorrhagic component due to iatrogenic hypocoagulability. A review of the literature is included.
