Emergency Department: The Need for New Organization Models

Many reasons were pointed out trying to explain the crisis seen
in the Emergency Services on January 2015. Surely this was a
hard winter and the work at the Emergency Services is always
difficult and chaotic. However, Internal Medicine doctors should
work on the solution for actual problems in the Emergency Services. The organization models currently working at the Emergency
Services have led to the breakup of each hospital into two different ones, one by doctors working on the emergency room and
another by doctors working at the wards. Emergency Services
teams, initially composed with competent and motivated doctors,
have been worn-out by the difficult and exhausting work, leading
many times to the rupture of their own teams, with the consequent
loss of quality on their work. Ward teams are currently confined to
night and weekend shifts and have learned to see the Emergency
department has a non pleasant place to work. Resident doctors
have also lost the feeling that the Emergency Services is a place
to learn. Reorganization of hospital teams can lead to a better
performance of Emergency Services. The implementation of a
mix model, including doctors of emergency and ward teams, may
have advantages for both groups of doctors and for residents. It
is necessary to discuss and implement new measures, preparing
for the next winter.
