Neuronopatia de Células Granulares num Doente com VIH e Recuperação Imunitária: Relato de Caso

A reativação da infeção pelo vírus JC associa-se classicamente à leucoencefalopatia multifocal progressiva (LEMP) em doentes com compromisso imunitário grave, nomeadamente no contexto de infeção pelo vírus da imunodeficiência humana (VIH). Porém, outras apresentações neurológicas são possíveis, tal como a neuronopatia de células granulares do cerebelo, entidade descrita em 2005 e notável pelo seu tropismo pelo cerebelo. Apresentamos o caso de um homem com passado de LEMP no contexto de infeção crónica pelo VIH-1, com boa recuperação imunitária e supressão virológica sob antirretrovirais, que desenvolveu quadro progressivo de síndrome cerebelosa. Os autores relembram esta patologia, entidade pouco reconhecida e associada ao vírus JC, bem como os desafios inerentes ao seu diagnóstico e abordagem.
