Faz-se a análise das interrelações entre hipertenção arterial (HTA) e doença coronária pondo-se em evidência a importância da coexistência de outros factores de risco para ate rosclerose e as explicações para o reduzido benefício obtido com a terapêutica anti-hipertensiva relativamente à doença coronária.
Refere-se o papel da disfunção endotelial in duzida pelos vários factores de risco nas alterações funcionais e estruturais das artérias coronárias.
Aborda-se afisiopatologia coronária na cardiopatia hipertensiva e focam-se os diversos mecanismos responsáveis pela isquémia, habitualmente subendocárdica, no coração hipertrofiado do hipertenso.
Chama-se a atenção para a importância que poderá ter a prevenção ou regressão da HVE e a diminuição da fibrose intersticial com a terapêutica anti-hipertensiva na melhoria da reserva coronária e prevenção da insuficiência cardíaca. Referem-se, por fim, as medidas terapêuticas que poderão quebrar o elo entre HTA e doença coronária.
