AGE – Avaliação Geriátrica Estruturada: Um Projeto de Melhoria da Qualidade no Serviço de Medicina Interna

Resumo

Introdução: O internamento é uma oportunidade para avaliar
de forma estruturada a vulnerabilidade dos doentes idosos.
Apresentamos o projeto de Avaliação Geriátrica Estruturada
(AGE) implementado no Serviço de Medicina do Centro Hospitalar
do Porto.
Métodos: O AGE caracterizou-se pela aplicação de uma
escala de rastreio de vulnerabilidade (VES-13) a todos os
doentes idosos internados, seguido de uma lista de verificação
de indicadores de qualidade retirados do documento de
consenso Assessing Care of Vulnerable Elders – 3 (ACOVE-3),
aplicados a esta realidade e suportados por instrumentos
de avaliação, tais como o Mini-Cog e o Mini Mental State
Examination (MMSE). Numa segunda fase do projeto, desenvolvemos
com as equipas de enfermagem, prospetos
para o ensino do doente e cuidador, dedicados a problemas
comuns no cuidado domiciliário do idoso vulnerável. Do
mesmo modo incorporamos uma lista de verificação de condições
pré-alta para os profissionais e outra para o doente.
Resultados: Apresentam-se os resultados de implementação
do primeiro trimestre da segunda fase do projeto, que
incluiu uma amostra de 543 idosos, tendo sido aplicada a
escala VES-13 a 300 desses doentes, dos quais 237 (79,0%)
foram identificados como idosos vulneráveis. Foram rastreados
83 (35,0%) doentes para demência e identificados 24
(52,2%) novos casos suspeitos de síndrome demencial.
Conclusão: Os autores consideram o projeto AGE um exemplo
de estratégias de melhoria de qualidade nos cuidados
aos doentes geriátricos internados que permitem alterar os
comportamentos da equipa de saúde.
