Pneumococcal bacteremia as the initial presentation of multiple myeloma – a case report

Multiple Myeloma (MM) accounts for approximately one percent 
of all cancers and is characterized by the neoplastic proliferation of a single clone of plasma cells producing a monoclonal 
immunoglobulin. 
Clinical features are anemia, bone pain, renal failure and hypercalcemia. Rarely, a bacterial infection is the initial presentation 
of this disease. 
We report a case in which bacteremia caused by Streptococcus 
pneumoniae, possibly associated to Septic Arthritis (SA), was the 
initial presentation of multiple myeloma. This situation has been 
seldom reported in medical literature. 
Association between pneumococcal infection and a specific 
predisposing disease is common. Pneumococcal infection should 
lead to a suspicion of underlying illness and the co-existence 
of other diseases must be ruled out in the diagnosis, namely 
multiple myeloma.
