“Hospital at home” is an innovative model that provides hospital-
level care in a patient’s home as a full substitute for acute
hospital care. It was tested at medical centers across several
countries. This care model lowers costs, reduces complications
and is highly rated by patients. The program is offered to patients
who require hospital admission for certain diseases, such as
community-acquired pneumonia, congestive heart failure,
chronic obstructive pulmonary disease, and cellulitis. Patients
who meet specific medical eligibility criteria can receive hospital-
level care–including diagnostic tests and treatment therapies
from doctors and nurses–in their own home. In Portugal home
care is very poor and hospital wards are often crowded with sick
frail elderlies, who would benefit from better health care in the
comfort of their home.
