Unusual form of presentation of an alcoholic liver disease

Abstract

The authors presenta case of a 38 years old man admitted to the Medicine I department of the Coimbra University Hospital for a left thigh hematoma. The patient was a alcohol heavy drinker and the physical examination showed a large thigh hematoma, jaundice and hepatomegaly. Laboratory findings revealed a severe coagulopathy. By lack of efficacy of medical support treatment the thigh

hematoma was submited a surgical drainage. After few days the hematoma was formed again anda radiolabeled erythrocyte scanning detected alarge and tortuous umbilical vein. An abnormal umbilical-iliacvein shunt detected by ultrasonography and arteriography explained the reason for the appearance of the thigh hematoma. The patient was proposed for liver transplant but died in the pos­ operatory of porto-cava systemic shunt that should precede thetransplantation.
