Isolated left ventricular noncompaction (ILVNC) is a rare cause of dilated cardiomyopathy, with dilated left ventricle, and exuberant trabecular pattern. One of the manifestations is thromboembolic phenomena, with prevalence of 21% - 38%. We describe the case of a 37-year-old woman whose ischemic stroke was the presentation form of isolated left ventricular noncompaction. She underwent thrombolysis with complete reversal of the deficits. ILVNC was detected in the study, in addition to patent foramen ovale. The cardioembolism was admitted as the cause of the stroke, leading to the decision of oral hypocoagulation. We emphasize ILVNC as an uncommon etiology of stroke and the importance of the etiological study of stroke in young patients, because it influences the therapeutic decision and the prognosis.
