Idiopathic retroperitoneal fibrosis: atypical evolution

The authors present a clinical report of a 68 year old woman, who
had spontaneous clinical and laboratory amelioration of serious
acute renal failure due to idiopathic retroperitoneal fibrosis, which
was demonstrated on histologic examination of a surgical biopsy.
Therapy with corticosteroids and implantation of bilateral ureteral
stents were decided in view of laboratory signs of persistence of
disease activity. The clinical and laboratory evolution was good
and ureteral stents were removed at four months duration of
corticosteroid therapy. A slow and progressive lower dosage of
corticosteroids do not caused a recurrence of the disease, but
due to the persistence of imagiological signs of ureteral dilation,
azatioprine was associated to corticosteroids, trying to maintain
stabilization of the disease without needing to recur to ureterolysis surgery.
The authors made bibliographic review of data on idiopathic
retroperitoneal fibrosis, focusing on etiopathogeny, clinical presentation and diagnosis, therapy and prognosis.
