Multiple Myeloma IgM – a clinical case report

The IgM monoclonal gammopathy can be associated with multiple
pathologies namely lymphoproliferative conditions, multiple myeloma, solid tumours as well as benign conditions such as infections
and monoclonal gammopathy of undetermined significance.
The differential diagnosis is not always easy and it should follow
clinical, laboratory and histopathologic criteria.
To this purpose the authors present a clinical case of a 56-
year-old male patient, with bone pain, located predominantly in
the right shoulder. The association of multiple osteolytic lesions,
monoclonal gammopathy IgMk and plasmocytic infi ltration seen
on the bone marrow biopsy established the diagnosis of Multiple Myeloma IgMk.
Theoretical considerations are discussed about this myeloma
subtype, considered by some authors as an intermediate form
between Waldenström’s Macroglobulinaemia and Multiple Myeloma.
