Sépsis: O Bom Senso está de Volta

A prevalência da sépsis aumenta em todo o Mundo, mas a sua abordagem continua desafiante porque não tem critério de diagnóstico nem marcadores específicos, além de que as definições têm sido alteradas. A sépsis 3 contribuiu para essa confusão: pois em vez de 1 propôs 2 critérios diferentes (qSOFA e SOFA), conforme o sítio em que se encontra o doente, com descritores diferentes para o mesmo parâmetro. Ignorou o conceito de SIRS e optou pelo reconhecimento dos quadros mais graves e com maior mortalidade, em vez de se centrar no reconhecimento precoce.
As Recomendações Surviving Sepsis Campaign 2021 (RSSC 2021) repõem o bom senso, recusam o qSOFA, reconhecem a importância do conceito SIRS no reconhecimento precoce e das estratégias de avaliação e tratamento centradas na clínica. É o território de eleição dos Internistas, no ambulatório, na urgência e nas enfermarias O presente texto tem a intenção de rever as componentes das RSSC 2021 que mais beneficiam da intervenção dos Internistas e da Medicina Geral e Familiar com base nas intervenções que documentadamente salvam vidas na sépsis.
