Abordagem terapêutica da hemorragia intracerebral na fase aguda

Resumo

A hemorragia intracerebral (HIC) corresponde a cerca de 10 a 15% das admissões hospitalares por acidente vascular cerebral, sendo mais de duas vezes mais frequente do que a hemorragia meníngea. Apresenta morbilidade e mortalidade muito elevadas, sendo maior nas primeiras horas e, por esse facto, considerada uma emergência médica. A abordagem terapêutica destes doentes tem sido alvo de muita controvérsia. Devido à ausência de estudos credíveis, randomizados, as atitudes terapêuticas não têm sido uniformes e provavelmente também não têm sido as mais correctas, com prejuízo óbvio para os doentes. Neste trabalho faz-se uma revisão da terapêutica da HJC baseada em guidelines recentes, focando os aspectos terapêuticos mais controversos, a sua etiopatogenia, as manifestações clínicas, o dia­ gnóstico e os factores de prognóstico.
